NEW ATHLETIC FORMS:
NEW P.I.A.A.
POLICY: A comprehensive physical is good for an entire school
year. The physical date must be after June 1, 2008. Physical
packets will also be available in the Main Offices of the 7th,
8th, 9th, and 10-12 Grade Centers by Tuesdays,
May 20. Forms (in red) can be downloaded below:

INITIAL PHYSICAL PACKET
INFORMATION: (Click

RE-CERTIFICATION PACKET
INFORMATION:
A student who participates in
more than one sport season in a school year and who got a
Comprehensive physical done at school or at their family doctor will
be required to complete the Re-certification Form later in the year
when it is time to go out for the next sport.
The only other required forms at that time will be the

The PIAA has officially announced a change in policy for all sport
physicals, effective June 1, 2008. Due to the
change in policy by the PIAA -
One physical
(dated
after June 1st)
is good for entire school year.
(See Re-Certification Information* for students participating
in more than one sport season.)
June 3, 10 and 11th are the dates scheduled for physicals for all
school-sponsored fall, winter and spring sports for the
school year 2008-2009.
The
cost of the
school sport physical is $20.00.
Checks
are made payable to: Dr. Wade Brosius.
A
physical will not be performed unless payment is received at the
time of physicals.
Students should wear or change into shorts and t-shirt for the
exam.
If you plan to
get a physical with your own family doctor, please note the date of
the physical
must be NO earlier
than June 1, 2008 and your doctor must sign at the
designated places on Section 4 - PIAA Comprehensive Physical Evaluation Sheet.
Only the PIAA Comprehensive Physical forms will be accepted for the
sport physical. These forms are in our physical packets. These
packets can be picked up in the schools where the physicals are
being held or you can download them above (in red).
Students who plan to participate in
athletics during the 2008-2009 school year should attend the
physicals in June.
There will be limited time slots for physicals in October and
February for those students requiring
Re-Certification
(see other side) or those unable to attend the June physical who
are only playing a sport in winter and/or spring.
Once again, we recommend that students going out for sports in
any season attend the June dates.
With this change
by the P.I.A.A., we ask that everyone please be patient when
attending your team’s scheduled timeslot. The scheduled times
we have listed are based on experience from other schools that have
performed this type of physical.

Comprehensive Physical
Forms
(Section 1 through 4):
Section 1, 2 & 3:
All forms must be filled out and signed by parent/guardian and
student, as indicated.
IMPORTANT: On Section 2 -
Parents should sign by
ALL sports in which athlete might have an
interest in
participating during the school year; otherwise, when
the season arises, if the parent
signature is not
there, the athlete will not be able to try out on the first day.
Section 4:
Physician needs to fill out
completely and sign. Please be sure the physician
has filled out all of the information required on the top portion
including name, age, school, sports (any your child might
want to participate in), height, weight, pulse, etc., and the
bottom portion (including typed or printed name, address, license #,
MD or DO and date of physical).
*Re-Certification
Information:
A student who participates in more than one sport and who
has a current comprehensive physical will be required to complete
the Re-certification Form – Section 5 & 6. (You do not need
to get a doctor’s signature unless you answer “Yes” to the
questions listed under the Supplemental Health History), The only
other required forms are the First Aid Kit Emergency, and
Eligibility
Information Sheet.
These packets will be given out at the sports meetings before the
winter and spring seasons or can be downloaded from this web
site.
For those student athletes who go to their own doctor or did not
turn the paperwork into the school the night of physicals: The
forms for the high school athletes should be returned to the
Athletic Office at the high school by August 4.
For athletes in grades 7, 8, or 9th, the physical
packets can be returned to the main office at the appropriate school
two weeks prior to the start date of the 7, 8, or 9th
grade sport. (We need time to compile all the information and
distribute it to the appropriate buildings and people.) If mailing
packets over the summer, please send to: Spring-Ford Senior High
School, Athletic Office, 350 S. Lewis Road, Royersford, PA 19468.
Physical forms are available at each school (7th, 8th, 9th, or
10-12th Grade Center) or on this website page.
