KINDERGARTEN REGISTRATION

2008-2009 SCHOOL YEAR

The Spring-Ford Area School District is currently gathering names for Kindergarten registration for the 2008-2009 school year.

AGE REQUIREMENT:

All Children Who Will Be 5 Years of Age by August 31, 2008

REGISTRATION DATES:

Brooke Elementary School – March 17, 18, 19

Evans Elementary School – March 17, 18, 19

Limerick Elementary School – March 17, 18

Oaks Elementary School – March 17, 18

Royersford Elementary School – March 11, 13, 26

Spring City Elementary School – March 17, 27

Upper Providence Elementary School – March 17, 18, 25

In addition to the dates above, Kindergarten registration

will handled by appointment at your child’s respective building

until June 1, 2008.

After June 1, 2008, registration will be handled at the Administrative Office located at 199 Bechtel Road in Collegeville.

Please complete the Kindergarten Registration packet and take it with you to your registration appointment.

All registrations are by appointment
You must bring the following items to registration
REGISTRATION PACKET
BIRTH CERTIFICATE
PROOF OF RESIDENCE (DEED, LEASE)
PROOF OF COMPLETED IMMUNIZATION:
Four (4) DPT shots (including one dose administered after the fourth birthday)
Three (3) doses of oral polio vaccine or four (4) doses of inactivated polio vaccine
Two (2) doses of measles, mumps, and rubella vaccine (the first dose administered AFTER the first birthday)
Three (3) doses of hepatitis B vaccine
Varicella vaccine (Chicken Pox) or proof of disease
