 Pennsylvania Advocates for Nutrition and Activity (PANA)  works collectively to make it easier to be healthy in the places we live, learn, work and play by changing environments to support healthier eating and physical activity options. PANA is administered by the Penn State Hershey Center for Nutrition & Activity Promotion.

Medical Forms

Medication Guidelines - Completed when your child needs to take medication at school.

Medication Field Trip Form - Must be completed when any medication, which is ALREADY AT SCHOOL and taken on a regular or as needed basis, will need to be taken during the hours of a field trip.

Absence & Tardiness - To be returned to school the day your child returns.  A phone call is still required on the day of the absence (610-705-6009, option 4).
