Please bring the following to your transition meeting:

1. Your child
2. A copy of your child’s current records from MCIU, if a release has not been signed
3. Any outside psychological or medical evaluations
4. Kindergarten Registration Documents - click here

WHO will be in attendance?

• Case manager from MCIU
• Spring-Ford Area School District Special Education Supervisor
• School Psychologist
• Speech/Language Pathologist

WHAT will we discuss?

• Your child's strengths and areas of need
• Current programming through the MCIU
• The evaluation process and what to expect moving forward
• Permission to Evaluate/Prior Written Notice will be issued
• Consent for Electronic Transmission of Documents

WHERE is the transition meeting held?

• Virtual via zoom

WHEN is the transition meeting held?

• Meetings are held December & January
• MCIU case manager will contact you to schedule your meeting

HOW long are the transition meetings?

• You will schedule a 30 minute group time slot
• Within that time slot, there are 4-5 tables (MCIU, Psych/SLP, Registration, Supervisor) that you will rotate through

WHY is this important?

• Our goal is to build a collaborative and trusting relationship with families
• Our goal is to support you throughout this transition so that we can appropriately program for your child when he/she attends kindergarten!

1. Permission to reevaluate is issued and parent gives consent. 
2. The reevaluation is conducted within 50 calendar days from the date the signed permission is received by Spring-Ford. 
3. Next Steps: Is the student eligible for Special Education Services? 

• If YES, parents participate in the IEP team meeting up to 30 days later to determine services for the child.
• If NO, the student is not in need of specially designed instruction; however they require accommodations through a Chapter 15/Section 504 Service Agreement. OR;
• If NO, the student is not in need of specially designed instruction. Their needs can be met in the general education setting.
