The Commonwealth of Pennsylvania School Immunization Law (Pa Code 23.83) requires children enrolled in school to provide evidence of proper immunization for certain diseases within the first 5 days of school.

The second dose of the Meningococcal conjugate (MenACWY) vaccine is required for entry into 12^th grade, the first dose would have been given to your student for entry into 7^th grade.

Proof of completion of administration of the second dose in the MenACWY series, a medical certificate documenting the scheduled dose or an exemption form MUST be received by the 5^th day of school without exception or your child may not attend school after the said date. *12th grade students will not be able to purchase a parking pass until this obligation is met.

Please use the Spring-Ford Medical/Religious Exemption Form if your child does not receive immunizations for medical or religious reasons.

The state of Pennsylvania mandates all 11^th grade students to have a physical examination on file. Private physical exams with updated immunizations dated on or after July 1, 2024 will be accepted for the 2025-26 school year.

Please visit the Health Services page for a copy of the blank Physical Form if needed. Please note that  the School Physical does not satisfy the PIAA Athletic Physical requirement. For information on the PIAA Athletic Physical, please visit the Spring-Ford Athletics page.  

*If your student is 16 at the time of their physical, they are eligible to receive the 2^nd dose of the Meningococcal conjugate (MenACWY) vaccine.  If your student is eligible, we encourage you to have this immunization administered and proof submitted to the nurse as soon as possible, as this vaccine is a state requirement for entrance into 12^th grade.   If this obligation is not met, students may be exempt from school and will  be unable to purchase a parking pass for senior year.

If your student receives a physical during their 10^th grade year, we may accept that physical for their 11^th grade physical requirement. Please see 11^th grade section above or contact the 10-12 nurses with any questions.
